About My Child

Summary

“About My Child” is a comprehensive and user-friendly caregiver report tool. The format empowers parents and guardians to share and understand their child’s functioning and health complexity. It collects real-time insights and fosters meaningful communication among families, clinicians, educators, and caregivers. It is appropriate for children from infancy to 18 years of age. A parent-report version is available for babies and toddlers (About My Baby/Toddler) and a self-report version is available for youth (About Me).
